Fees:

• Insurance co-payments or client payments are due at the time of each session.
• There is a fee of $50 for cancellations less than 24 hours in advance.

Insurance:

Treatment services are in network with the following insurance providers:

• Anthem (Blue Cross/Blue Shield)
• Aetna
• Cigna
• United Behavioral Health
• SIHO
• IU Health
• Cook Group Health Plan

If your insurance carrier is not listed you may still be entitled to coverage out of network. Call your insurance carrier for further information.
